Sponsor’s Name: __________________________________

“Rotary is an organization of business and professional persons united worldwide who provide humanitarian service, encourage high ethical standards in all vocations and help build goodwill and peace in the world.”

The prospective member has attended at least two regular meeting, as a guest of the club, on the following dates:

1. _________________ 2. _________________

Prospective member understands that attendance at the regular weekly meeting is a requirement. The goal is 100% attendance. While it is understandable a member will not be able to attend every meeting of their home club, the member is expected to “make-up” by visiting another club or on-line. The preferred method of making up is by visiting another club.

Non-attendance is a reason for having membership terminated; e.g., a member failing to attend or make-up four consecutive meetings is automatically terminated.

There is a one-time, non-refundable initiation fee of $50.00. Currently dues for the Rotary Club of Downtown Statesboro are $210.00 per quarter, which includes $25 contribution to The Rotary Foundation.

Members are expected to participate in club programs and projects.
I understand these basic items about Rotary and want my sponsor to pursue my membership into the Rotary Club of Downtown Statesboro.
